You check the water softener and see plenty of salt, yet soap stops lathering well, scale returns to faucets, or dishes develop spots. The equipment may not have failed. The salt inside the brine tank may have formed a hard crust with an empty space underneath it.

This is called a salt bridge. It can make a brine tank look full even though the water below cannot reach enough salt to make the brine needed for regeneration.
